Make Sure Your Health Forms are Up-To-Date
Kindergarteners and students new to Wescott are required to provide the school with an Illinois health exam form with an immunization record and a physical, which was completed within one year prior to the first day of school. Kindergarteners and new students will also need to hand in a completed eye exam and dental form. The forms may be found at District 30 Health Forms.
Second-graders will need to turn in a dental form this school year by May 2023. If your student will be keeping allergy or asthma medication in the health office, a current health care plan from your physician is required.
Before any medication can be dispensed, the school must receive written instructions and a signed permission form from the student’s physician and parent. All medications must be properly labeled and kept in the Health Office. Parental permission must also be obtained for non-prescription medication. Please contact the Health Office at 847-400-8760 or firstname.lastname@example.org if you have questions.
