Online Forms
At the beginning of each year we ask our families to complete necessary paperwork for the school year. This year, most of those forms need to be completed on-line in Power School.
All Parents (VIRTUAL AND IN-PERSON) MUST COMPLETE the Mandatory Forms NO LATER THAN Thursday, September 9th , 2021 for their student(s)
The instructions on how to access Power School Forms through a browser are below :
1. Log into Unified Classroom Parent Account (BROWSER ONLY – mobile app will not work) **Please note, If you are unsure of your password, please click on the "forgot password" and a link should be sent to the email address on file for you to reset it. Unfortunately, we do not have the ability to access or reset your password from our office. **
2. Click on Forms
3. Under General Forms click on "Annual Update"
4. Complete any forms that require changes (Update Student Address, Update Legal Guardian, etc.)
5. If there were no changes, you still must complete the “Required to Complete Each Year” which confirms that all information is accurate.
6. Complete any additional forms below (e.g. Technology Agreements)
7. Submit the form
If you have another student, please repeat the process for them.
Since this process is new to us, we will be printing off your child's emergency form and asking you to make any changes to it, just as a double check to ensure we have the correct contact information. If, when you receive your child’s paper copy of their emergency card you see that it does not reflect the changes you made online, please take a moment and update it.
I know this may seem like an additional step, but with 500+ families making changes it will take a bit of time for Mrs. Constantineau to go through and approve the changes in the system. (Please note, you do need to update the form in Power School, if you don't, then we will have to hand update all the changes for each of our students.)

Attendance Policy
Your child will be excused from class for illness, bereavement, recognized religious holidays and medical or dental treatment that cannot be scheduled after school or on weekends.
Your child is responsible for makeup work.
Homework requests may be made for an absence of two or more days in the case of both excused and pre-excused absences.
Homework requests require 24 hours to process.
VACATION POLICY
Absence for family travel during scheduled school days is discouraged.
However, should family travel be necessary, parents must submit a written request to the teacher for a pre-excused absence at least one week in advance.
Arrangements to complete classroom assignments can be made with the teacher.
You can arrange for home study through the school principal should your child be absent from school for an extended period due to physical disability or illness.
LATE ARRIVAL
If a student arrives late to school, the parent must accompany the student into the office, and the student must be signed in before he or she can go to class.
Truancy officials will be contacted in the event of a student having excessive tardies or absences.
Free and Reduced Lunch Applications
Families, it is important that free and reduced meal applications are filled out for the 2021-2022 school year. School meals are free of charge to all UCS students for the 2021-2022 school year, however families that qualify for free or reduced meals still need to fill out the annual form to benefit from all this program provides. Title I funding is also tied to meal applications. The attached flyer outlines the benefits available to qualifying families who submitted an application. Forms can be filled out online at www.lunchapp.com or by using paper applications available at your school.
Policy On Personal Items
Any cell phones must be turned off and out of view during school hours or while on the bus OR IT WILL BE CONFISCATED and an adult must come up to school to pick it up.
Students should NOT bring toys, game boys, Nintendo DS or any hand-held electronic games, CD players, MP3 or IPods, PDA’s, memorabilia, special cards or other collectible or expensive items to school.
Havel Elementary School is not responsible for personal items that are lost or damaged.
Such items will be collected and kept in the office. A parent must come and retrieve any items they wish to have returned.
Birthday Celebrations
The teachers and staff at Havel strongly support and encourage the recognition of a child’s birthday during the school day. It is an annual milestone in every child’s life that should be celebrated not just at home, but in school as well, where children spend one-third of their day with teachers, classmates, and peers. While there are many wonderful and creative ways to celebrate a child’s birthday in school, birthday celebrations at Havel will not include food treats. Instead, the focus at Havel will be recognizing the day in a variety of special ways that do not include food treats. Some possibilities include:
· Class sings “Happy Birthday”
· Happy Birthday Recess/Gift
· Morning “Happy Birthday” PA announcement
· NEHS Singing Telegram
· Parent makes a donation of book to class library/read to class
· Student reads favorite book to class
· Non-Food treat bags for the class
· Parent/special someone reads story to class
Your child’s teacher will have more information on how birthdays will be celebrated in the classroom. As educators, caregivers and loved ones we all want the best for our students. Thank you for joining us in giving students healthy opportunities to celebrate important events and achievements.

Student Devices
- All new students (1st-6th grade) will be issued a device sometime in the first week of school.
- Kindergarten students will be getting their iPads toward the end of September.
- 2nd Graders will be turning in their iPads and getting a laptop.
- Any student with a damaged or older computer will be getting new devices. Havel staff will assist with determining which computers will need to be replaced.
- Don't forget to read the information below regarding purchasing optional insurance. Families will be responsible for any damages to the iPads or laptops.

Transportation Note
You may see our buses out and about in the community at this time as our drivers are completing test and practice runs to establish familiarity before school begins. We thank you in advance for your patience as some consistencies, particularly stop times, will need the first few days of school to develop routineness.
Until runs become established, you may estimate the approximate stop times for your location by applying the following general practice: buses for elementary schools with 9:20 a.m. starts will begin 25 minutes prior to the start of school. The full routes are available in detail on our website at http://www.uticak12.org/busroutes.
Please note - Through a federal CDC order, Macomb County is requiring that face coverings are used on all public transportation, which includes families who choose to use our bus services. This is the same national transportation order that you may have experienced if you flew on an airplane this summer.
As always, we ask that students arrive at the stop location ten minutes early to account for changing daily factors such as traffic even once the runs have been established.

Before your child leaves in the morning, ask these two questions:
o In the last 24 hours, has your child developed any of the following symptoms that are new/different/worse from baseline of any chronic illness: cough, shortness of breath, difficulty breathing, new loss of smell and new loss of taste?
o In the last 24 hours, has your child developed any of the following symptoms that are new/different/worse from baseline chronic illness: subjective fever (felt feverish) or measured temperature 100.4 degrees Fahrenheit or higher chills or rigors (sever chills with shivering), headache, sore throat, muscle aches, nausea, vomiting, diarrhea, runny nose or congestion and fatigue?
The answer should be no to both questions. If not, your child should remain at home for their protection and the health and safety of his/her classmates and the adults in your local school. Please call your school’s office to communicate this absence.
