504 Reminders
We are going to start holding our annual 504's soon so you will start to see teacher input forms showing up in your boxes. They are going to look different from what you used last year. I used this teacher input form at York and we loved it because it got straight to the point and was not time consuming at all. We're hoping you guys see it the same way.
A couple things...
- We are not scheduling the 504's until we get teacher feedback and have a chance to talk with the parents so the "date of meeting" blank will not have anything written there.
- Number 1 asks for reasons why they may be struggling. If they are not struggling you can leave them all blank except for Current Grade, we would like to know what their current grade is either way.
- Number 2 will ask you to draw a line through any accommodations the student doesn't use (Accommodations are on back) and put a check mark next to those that should remain. Then, you can add any accommodations you are providing, that are not listed already, so we can include them.
- Number 3 is for anything else you would like us or the parent to know. At the bottom you will find STAAR accommodations. If you have a student that can handle Oral Admin online please let us know.
- When your done please return them to Janet Walker by the specified date (top left of the document).
If you have any questions we are here!
