Information from Our School Nurse
This document was first sent out in the beginning of the school year. We want to remind our families that this checklist is on the virtual backpack and the school district website. Click here for the link to complete Health Information document. The Daily Health Monitoring Checklist can be seen below and is under Quick Links on the MA website.
People often believe that a fever is the presenting symptom for COVID-19 and that if your child does not have a fever, then it is not COVID. This is not the case or trend in the pediatric population.
Thank you for your continued compliance with health and safety protocols.
Health Requirement Reminders:
All Grade 6 students are required to turn in a Physical Exam Form.
All Grade 7 students are required to turn in a Dental Exam Form.
Completed forms should be sent to Mrs. Donna Liberg at dliberg@lmtsd.org.
Attendance Information
We have updated our procedures for daily attendance.
All of your attendance reporting needs for all learners (hybrid and virtual students) will be handled by one electronic form that will replace the Blue Card online form and the Yellow Card online form (used at Pine Road). This will also replace the need for a phone call to the attendance line.
The MA Attendance Form is posted under Quick Links on our website and will be used to do the following:
-Use this form to report your child’s absence. Submitting this form will both report the absence and document it. No follow up Blue card is needed.
-Submit this form to inform us that your child has a change in his or her learning location (for example, will be learning from home instead of coming into school on a scheduled hybrid day).
-Submit this form to sign your child into school late.
-Submit this form to sign your child out for an appointment (early dismissal). This form allows you to enter a return to school time if your child will be returning to school.
Please Note:
If you are taking an extended vacation, you will still need to complete the Request for Excusal from Regular School Attendance, familiar to most families as a vacation request form. The form is found on the Virtual Backpack under Forms.
Doctors’ notes still need to be submitted to the school attendance secretary or sent into school.
Picture Day and Health Screenings for District Virtual Students
Virtual students (at PR, MA, and LMHS) will have the opportunity to have their pictures taken at the Murray Avenue School on Tuesday, November 3rd, 2020. Families who would like to get their child’s picture taken can sign up for a time slot using this link by October 30, 2020. We encourage all families to have their child photographed for school recordkeeping and the yearbook. Families who would like to purchase pictures will pre-pay online. This information will be sent as we are closer to the date of this event.
We are also doing required health screenings on this day. Students will have their pictures taken and then enter into a private area to do height/weight checks, vision and hearing screenings. These are screenings completed during the school year by our school nurses.
Families should park in the Murray Avenue parking lots (back lot by the Upper Gym or the front lot) and enter from outside into the Upper Gym.
MA Picture Retakes/Make Up for Hybrid Students
There will be a picture retake (or make up) day on Thursday, November 12 for Blue hybrid students and Friday, November 13. Students who want a retake should bring in their photo. Students who were absent for picture day will be given what they need at school. The order form is on the virtual backpack.

Technology Use and Expectations
Technology Reminders:
-Please email helpdesk@lmtsd.org if you are experiencing technology issues that you need help resolving.
-Please remind your child to restart his or her Chromebook every day. This resolves many technology issues.
-Please refer to the Parent Information Update 9-12-20 letter that explains expectations for student behavior on Google Meets.
-All teachers are using Go Guardian to monitor student online activity for both in person and virtual learners during school hours. This software also helps us resolve attendance issues, when a student's attendance is in question. Students should use the district Chromebook when working at home so that this monitoring software can be used. This software allows teachers to close extraneous tabs and send students messages during class to redirect their work if needed.
