Youth Truth Staff Survey
The Staff Survey is:
- Available to administer between 03-11-2019 and end-of-day 05-31-2019.
- Taken online
- Available in English & Spanish
- Anonymous
- Will take approximately 15 minutes to complete
- Suitable for all staff members - including both instructional and non-instructional school staff.
Login Codes Available Soon!
A login code is a randomly assigned alpha-numeric code that each staff member must enter in order to gain access to the survey. Each login code is made up of 6 letters/numbers. The login code identifies which school each staff member is from (though in no way identifies the individual) and also allows YouthTruth to maintain the security of the survey site.
Any staff member can receive any login code (the staff feedback survey should be taken by all staff members - both instructional and non-instructional).
Login codes are one time use only and should be discarded once used. Please do not give the same login code to more than one staff member.
If a staff member starts the survey, but doesn't finish in one sitting, he or she can pick up where they left off by entering their same login code. Once the 'submit' button at the very end of the survey has been clicked, the login code will be rendered unusable.
