HORIZONS Parent Surveys
If you are a 1st – 5th grade parent and requested your student participate in mid-year HORIZONS testing, a parent survey was sent home. Please complete the survey and return it with your student as soon as possible. You must complete in the survey in order for your student to participate in testing. If you would like your student to participate in mid-year testing, you have until November 30th to fill out the google form at the QR code below. If you are a parent to a Kindergarten student, you do not need to request screening. Mid-year Kindergarten screening will occur for all students.
