Language Assistance Program Update
October 13, 2017
Documents in the Cumulative File for English Learners
Home Language Survey - The original HLS form that was filled out when a student first registered to attend an Indiana school should be in the cum folder for all students (ELLs and nonELLs.) This form is filled out only once. If a language other than English is marked on the three student-specific questions, that student will be assessed using the WIDA Online Screener or Kindergarten Screener, and the student's scores will be used to determine placement for ELL services. Students with overall scores between 1.0 - 4.9 qualify for services as an English Language Learner.
Test Scores - WIDA Screener scores and writing test, if applicable, for newly identified ELLs or the previous two years of spring WIDA ACCESS scores as well as ISTEP+ scores
A copy of any dated letters to parents, including the Parent Notification of Placement or Continuation Letter - A letter is sent home annually for students who qualify for ELL services.
Completed Monitoring Forms - for students who have exited the program in the last two years
Individual Learning Plans for your ELLs - a list of accommodations and modifications for English Learners.
Yearly Updated Denial Forms - if parents decline ELL services (the student will still have an ILP and will receive accommodations and modifications, and must take WIDA ACCESS test annually.)
