Illinois 5Essentials Survey
We invite you to provide feedback to our schools by completing the 11th annual Illinois State Board of Education 5Essentials Survey. This survey is administered by UChicago Impact at the University of Chicago. Students, teachers, and parents provide their opinions related to five indicators that can predict important student outcomes. Our principals use the survey results to help guide their improvement efforts at the individual schools.
To take the survey, please visit https://survey.5-essentials.org/Illinois and select the appropriate survey to begin.
