Message From Central Road School
Mrs. Joanna Shostachuk, School Principal
A Note from Mrs. Shostachuk 8/15/2021
Dear Central Road Families:
It is hard to believe that summer has offically ended. We are VERY excited to be welcoming back students into our building this week! We also understand this is a transitional time for families as we get back into our school routine(s). Please let us know if you need anything during this transition. Our teachers and staff can’t wait to engage students in academic and social and emotional activities as we begin settling into our new learning year!
The Central Road staff would like to invite all of our students to Meet the Teacher on Tuesday, August 17th from 8:30-9:30 am. We ask students to bring their school supplies to Meet the Teacher. We will be selling assignment notebooks outside (weather permitting) for $5.00 each. Any school forms or any needed medications can be dropped off as well. If you and your child are not able to attend our scheduled Meet the Teacher, your child may bring his/her supplies on their first day of school. School supply lists have been linked below for your review. We continue to monitor current Covid-19 mitigations. We ask for families to please be mindful of the number of family members attending. We continue to work to ensure a safe and healthy school environment. If you are not feeling well or display any symptoms of illness, please remain at home. All adults and children entering our building on this day are expected to wear a mask.
Please kindly remember to send your child to school with a mask and their self-certification lanyard each day. We hope to distribute them at Meet the Teacher but we are waiting for their arrival to our buiding. Students are welcome to bring a water bottle for drinking water. Our water fountains have been turned back on for refilling water bottles as needed.
Free, optional breakfast will be served in classrooms upon entry each day to students who would like to participate in breakfast this year. We will not be able to send home breakfast for the week like last year. Breakfast will be served between 8:30-8:45 am and our school day will promptly begin at 8:45 am. To limit exposure to both allergens and SARS-CoV-2, students will not eat snacks in the classrooms again this year.
3rd and 4th grade students will receive 1:1 chromebooks this year. These chromebooks will be distributed on the first day of school. K-2 chromebooks will remain at school. Students in grades 3-6 will be able to take their chromebooks to/from school as needed.
At last week's Board of Education meeting, Dr. Heinz shared D15's Return-to-Learn, Reconnect, and Re-engage Plan. Please take a moment to review it at your convenience as it holds a great deal of important information as we transition back to school.
We look forward to a safe, healthy, productive, fun, and meaningful school year filled with amazing learning and growing! Our first week back will be filled with lots of first week of school memories!
Kindest Regards,
Joanna Shostachuk

A Note from the Central Road Health Office
As we begin school in a few days, it is time to make sure your child’s health requirements are up to date.
All students new to District 15 will require a current physical exam with all immunizations up to date (Certificate of Child Health Examination), and students new to the State of Illinois will require an eye examination. This form is available in the school office, online, and here: State of Illinois Eye Examination Report.
All students should provide proof of immunizations by October 15, 2021, or the student will be excluded from school. The list of current immunizations can be found on the district website here: IDPH Health Requirements.
Students with health conditions such as asthma, epilepsy/seizures, severe allergies, and diabetes require the emergency action plan completed by their physician along with the Medication Administration Forms, signed by the physician and parent, for any medications required at school. Please bring these forms and medications on or before the first day of school. These forms are available in the school office, online, and here
Diabetic Medical Management Plans must be brought in along with medication and supplies prior to or on the first day of school in order for the student to attend.
If you have any questions regarding the above requirements, you may reach me at patela1@ccsd15.net or at 847-963-5102. Any questions related to COVID-19 may be directed to me in terms of your child having any symptoms or if your family/child has been exposed. We will work together to ensure all guidelines are followed and clearly understood while keeping our students all safe and healthy!
Thank you for your patience.
