Immunizations Required for School

Texas school immunization requirements are determined by the state legislature and set by the Texas Department of State Health Services (DSHS), in conjunction with the Texas Education Agency.

EMS ISD Health Services also encourages you talk to your Health Care Provider regarding any questions concerning your students immunizations. Below is information regarding local vaccine clinics.

Passport Health Back to School Immunization Clinics

Passport Health/Staying Healthy Medical Services will be conducting a children’s vaccination clinic to get your student attending Eagle Mountain-Saginaw ISD up to date for the school year.

The following forms of payment will be accepted:

Aetna, Blue Cross Blue Shield, Cigna, Humana & United Health Care PPO plans will be accepted.

**WE CANNOT ACCEPT AETNA & BLUE CROSS GOLD, SILVER, OR BRONZE PLANS OR HMO PLANS WITH ANY INSURANCE COMPANY.**

A paper photocopy of insurance card and driver’s license MUST be provided to receive vaccines at no charge.

Medicaid/CHIP

No charge for vaccines. Must provide a paper photocopy of card.


UNINSURED

$10.00 PER VACCINE ($40 maximum per child.)


CASH OR CHECK ONLY PLEASE

Vaccines available: Chicken Pox, DtaP, Hepatitis A, Hepatitis B, HIB, HPV, Meningococcal, MMR, Polio, Pneumonia and TDaP

A printed current vaccine record is required, and a Parent or Guardian must be present. (Electronic proof cannot be accepted. A paper copy must be provided.)

Due to COVID-19, only the child receiving vaccines and one parent/guardian will be allowed in the clinic area.

This clinic should not replace any well child checkups at the pediatrician.

Children who resist shots may not be seen.

Staff and supplies are limited. The clinic may be discontinued when supplies or time runs out.

Tarrant County Public Health Department provides immunizations at their outlying public health centers. Immunizations are available for children on Medicaid and CHIP as well as children who are underinsured (their insurance does not cover vaccines) or uninsured.

COVID-19 Vaccines

COVID-19 vaccines are now available to those ages 12 and older. The COVID-19 vaccine is not required for school attendance. You may want to consider vaccinating your student. Please reach out to your Health Care Provider with any questions you may have regarding the COVID-19 vaccine.
