Progress Monitoring

  • Please turn in your Medicaid progress monitoring graphs to Laura. These progress reports must include a signed and dated statement regarding progress for that trimester. If the student is NOT progressing, the statement needs to include what changes will be made to interventions to help the student progress. If the student has met or exceeded a goal then the statement needs to include what new goals that student will have.
  • All other progress reports need to be put into your case manager folder under progress reports in the the K drive.
