Note from the Patriot Clinic
Dear Parents and Guardians of Rising 7th Graders:
All public school students, including Forsyth virtual students, born on or after January 1, 2002 and entering, advancing, or transferring into 7th grade in Georgia need proof of an adolescent pertussis (whooping cough) booster vaccination (Tdap) given on or after 7th birthday and an adolescent meningococcal vaccination (MCV4) given on or after 10th birthday. Proof of both vaccinations must be documented on the Georgia Immunization Certificate (Form 3231) by the first day of school for all 7th graders.
Where can a student get vaccinated?
Children should visit their health care provider to get their Tdap or meningococcal vaccination. Local Health Departments also offer the Tdap and meningococcal vaccine.
What if I am unsure if my student has received these immunizations?
Please call your health care provider or school nurse.
Once I confirm my student has received these immunizations, how do I let the school know?
The school nurse has access to the Georgia Registry of Immunizations and will receive notification when your student's vaccinations are updated in the system. You are also welcome to email or call the school nurse at f39018@forsyth.k12.ga.us 770-781-4889 ext. 290120.

FAQs Related to Quarantines
Why am I notified days after my student has been exposed?
The administration begins working on contact-tracing as soon as we are notified of a positive case. Test results often come to families a day or two after the test is administered, and then we have to contact-trace 48 hours back from the on-set of symptoms. This creates several days of delay that we simply have no control over. We work as quickly as we can, but the contact-tracing process is also time-consuming, as we study classroom seating charts, take measurements in classrooms, and work with transportation for bus information.
If my child has already had Covid-19 but is directly exposed at school, does he/she have to quarantine?
No. If your student has tested positive within the last 90 days and the school has record of that positive test, your student does not have to quarantine.
