Check & Connect
I want to make you aware of a new call initiative, Check & Connect.
We at Columbus City Schools understand the negative impact that COVID-19 has placed on many of our families .Check & Connect is a way for us to hear about what you are experiencing in real-time and provide you with up-to-date information about social, health, and government services. It is also a way to remind you of upcoming district and building events.
I want to inform you that any call from the number (380)997-5960 will be a check & connect call.

IMPORTANT COVID VACCINE INFORMATION FOR ELEMENTARY FAMILIES
Thanks to our partners at Nationwide Children’s Hospital and Columbus Public Health, CCS will be able to provide dedicated COVID-19 Vaccination Clinics for students ages 5-11 at every CCS elementary school in December and January.
Parents you will only have to complete a COVID Consent Form; you do not need to be at the school when it is being administered. This is a great opportunity for you who may not have regular access to a health care provider or simply have not been able to schedule a time to get the vaccine for your children ages 5-11.
https://www.ccsoh.us/consentform5-11
The student vaccination clinics will be at every elementary school during regular school hours during the second week in December and the first week in January.
• First Dose: December 13-17, 2021
• Second Dose: January 3-7, 2022
Parents you must complete and return a COVID-19 consent form no later than December 7. Consent forms can be completed electronically on the CCS website or by paper copy and returned to the school nurse. Just to be clear - this vaccine clinic is just for our younger students. The vaccine will not be offered to parents or children ages 12 and older.
For a list of locations providing the COVID-19 vaccine to ages 12+, visit www.NationwideChildrens.org/Vaccine or www.gettheshot.coronavirus.ohio.gov.
