Where is the RTI process documented?
In Aware . . .
Go to STUDENTS . . . then FORMS . . . then RTI . . .
The RTI Process Documentation
- Teacher completes the Teacher Pre-Referral Baseline Information when concerned about student's lack of progress (academic or behavioral)
- Teacher provides intervention support and documents support given in IAP (under Student Academic Plan)
- Emails RTI Coordinator for grade level after minimum of 4 weeks of intervention support documented to schedule initial SST meeting.
- Teacher completes the 4a-SST Teacher Current Data Form to update information.
- At SST meeting, recorder completes the "2-SST Initial Committee Meeting" form and at subsequent meetings, the "4b-SST Follow Up Meeting."
- At SST meetings we will also need to update the MONITORING PLAN in the IAP to match the SST minutes.
