Attention 5th Graders!
Yearbook Cover Art Contest
You are invited to submit YOUR ARTWORK for the front and back covers of the 21/22 NHUF Yearbook! Two pieces will be selected – one for each cover. Every effort will be made to include all other artwork received in the yearbook.
Submissions must be on an 8½ x 11 piece of paper.The more colorful, the better! Remember to include your name somewhere on your masterpiece.Original artwork only please, no copies or printouts.
Artwork must be submitted by October 30th to be considered for the contest.Simply hand it in to the secretaries in the front office before the deadline!

From the Nurse's Desk
Below you will find information regarding health room procedures. If you have any questions, please feel free to contact the health room nurses at 610-473-1770.
Physical & Dental Forms: All state mandated Kindergarten and 3rd grade private dental forms and all Kindergarten private physical exam forms are due now. If you have an upcoming appointment, please notify the health room of the appointment and we will make note of the date in your child’s record.
Mandated Health Screenings: All current state mandates are still in effect. Students in grades K-5 will have their annual health screenings from now through December. If your child does not pass a screening, a referral letter will be sent home for you to follow up with a private physician.
Medication Information: If your child requires prescription or over-the-counter medication while at school, a medication authorization form (found on the BASD website) must be completed by your physician. Once completed, please submit the paperwork, and then contact the health room to schedule an appointment to drop off your child’s medication. Due to COVID-19 policies and procedures, parents will not be permitted to come into the health room to drop off medication, so it is necessary that you schedule an appointment prior to dropping off any medication. Prescription medications must be in an original pharmacy labeled container. Over-the-counter medication must be in original packaging. Please refer to the BASD website for more information.
Returning Paperwork: Please return any health room forms (Physical, Dental, Medication) via email if possible. You can scan and email the original document or email a photograph from your phone to either ewismer@boyertownasd.org or ekovach@boyertownasd.org
COVID-19 Information: Please ensure that you are monitoring all family members for symptoms of illness. See below for a list of symptoms consistent with COVID-19 and be advised that if a student presents with these symptoms during the school day, he/she may be sent home and require follow up with a physician.
· Fever over 100 degrees F
· Cough
· Shortness of Breath/difficulty breathing
· Chills
· Fatigue/muscle or body aches
· Headache
· New loss of taste or smell
· Sore throat
· Congestion or runny nose
· Nausea/vomiting/diarrhea
If your child is sent home from school due to symptoms of illness consistent with COVID-19, one of the following must apply before returning:
1. A negative PCR test result.
2. A doctor’s note with an alternate diagnosis AND no fever for 24 hours, without the use of fever reducing medication AND improvement of respiratory symptoms.
3. 10-day quarantine
Change of Clothing: Please consider keeping a change of clothing in your child’s backpack. Clothing can become soiled due to a bathroom accident, fall outside in the mud, or because of spilling food/drink. The health room may not have appropriately sized clothing, so if your child does have an accident of any sort, a parent will be called to bring clothing in for the child.

Annual Student Information Verification Reminder - Deadline - October 22nd!
If you have not completed your student’s Annual Student Information Verification, the verification window was reopened on September 22, 2021. You will have until October 22nd to complete your student(s) verification. For the safety and well-being of all BASD students, the District must have the student's most recent demographic, emergency contacts, and health information.
Below are instructions on how to complete your student's Annual Student Information Verification from within the Parent Portal.
If logging in from a computer, please use the steps below. Infinite Campus works best on a Chrome web browser.
- Log into Infinite Campus Parent Portal
- Click on More on the left side of the screen
- Click on Online Registration
- Click on "Click here to go to Existing Student Verification"
- Click on Begin Registration button
If logging in from the app on your phone, please use the steps below.
- Log into Infinite Campus Parent Portal
- Click on the 3 black lines in the upper left corner to view the drop-down menu
- Click on More on the left side of the screen
- Click on Online Registration
- Click on "Click here to go to Existing Student Verification"
- Click on Begin Registration button
An Infinite Campus Parent Portal account is needed to complete your student's ASIV. If you do not have a Parent Portal account, please contact your student's school for an activation key and instructions. Only one account per family is required. Please note, the activation key is person-specific. Only one account is needed per family; however, each parent can create their own account for the student(s).

Get your pictures in the yearbook!
You are invited to submit pictures to be considered for the yearbook. If you’ve got great photos of school events, the Yearbook Crew would love to see them! Just a few ideas of photos we want to see:
· First day of school
· Kona Ice truck
· Race Into Hibernation
· Band/Orchestra
· Chorus
· Safeties
· Fall Fest/Halloween
· 100th Day of School
· And anything else school-related you can think of!
Please get creative! Help us get as many photos as possible to create a book full of memories the kids will love.
HOW TO SUBMIT YOUR PHOTOS ONLINE

2. Enter User ID: 416875973
3. Browse to select the photo(s) you wish to upload, click on "Select My Images". (Note: only high-resolution JPEG, TIFF and PNG files can be accepted.)
4. Enter information about the photo and provide contact information in case a member of the Yearbook Crew needs to get in touch.
5. Check the box to agree to the terms.
6. Click "Upload Chosen Images."
It's that easy! Submit your photos today! Photos will be accepted until March 10, 2022.
