Students will remain in one or the other instructional model for a 6 week period. This policy is in place in order to provide instructional structure & consistency for the student.

It is necessary to fill out this form (see link below) if you are CHANGING instructional models for the next 6 week period.

For example:

1. If you are currently in face-to-face learning but want to change to At-Home Learning for the next six weeks, please complete this form. OR

2. If you are currently in At-Home Learning but want to change to face-to-face learning for the next six weeks, please complete this form.
