Student Self Certification Guidelines for COVID-19
The parent/guardian signature on the attached slips is verification that prior to your child boarding the school bus each day or entering a school building each day, your son/daughter has received a daily symptom screening at home by an adult caregiver and has verified your son/daughteris not experiencing any of the following COVID-19 symptoms:
-Fever (100.4 degrees or higher, without the use of fever-reducing medication)
-Muscle or body aches
-Shortness of breath
-Newuncontrolled cough(for students with chronic allergic/asthmatic cough, a change in their cough from baseline)
-Congestion or runny nose
-New loss of sense of taste or smell
-Diarrhea, nausea, vomiting, or abdominal pain
Additionally, the parent/guardian signatureis verification that priorto boarding the school bus each day and/orentering a school building each day, your son/daughter:
-Has no thad close contact with any person who has a confirmed or suspected diagnosis of COVID-19 within the last 14 days. CDC defines CLOSE CONTACT as:
-Anyone (with or without a face covering) who was within 6 feet of a confirmed case of COVID-19 (with or without a face covering) for a cumulative of 15 minutes throughout the course of a day.
-close contact begins two calendar days before the onset of symptoms
-if close contact was not showing symptomsthe period begins two calendar days before they tested positive
-Has not traveled outside of the country within the last 14 days
If your son/daughter WILL NOT BE IN ATTENDANCE FOR ANY REASON, please notify the school by calling the attendance office. Please include a listing of symptoms and/or reasons for the absence when speaking with the secretary or leaving a message, School staff may contact you to gather additional information. Attendance for Remote Days
IMPORTANT MEAL INFORMATION
Ordering Lunch for your student(s)
Every Friday, your teacher will send home a google form. At this time, choosing a lunch for your student on their at-home/remote learning days means your student(s) will receive breakfast and lunch. As of September 3rd, the USDA announced the extension of the "Free Meals for Kids" program which will allow all students to receive school lunches and breakfasts at no charge until the end of the calendar year.
**If you would like to have meals provided on school days Monday - Friday for the rest of the calendar year or while the "Free Meals for Kids" program is active, please email firstname.lastname@example.org with your child's name and classroom teacher. If you choose this option, you will not have to fill out the google form each week for your student(s).
The carry over period for students who were receiving free/reduced lunch for the 2019-2020
school year will be until October 20, 2020. If you havenn't turned in a new 2020-2021application or received notification for approval for this school year, your child will automatically be charged for a lunch as of October 21, 2020. Free/reduced lunch applications for the 2020-2021 school year are available in the office of each school or on the Sandwich School District website at https://www.sandwich430.org/parents/forms. Please note, even though free meals will be provided to all students through December, we are still urging those that normally apply for this program to still apply. This way when the USDA program ends your student will still be provided with free or reduced meals if approved.
Change in Transportation
When there is a change in your student's transportation, we prefer an email notice to your student(s) teacher. In an emergency, please contact the school office no later than 2:00 p.m. so we can notify our staff and ensure your child's safe dismissal. If the school does not have a notice of change in transportation, the normal way of transportation will be followed.
*When dropping students off in the parking lot, we ask that everyone stays in line to exit the parking lot for everyone’s safety. Please do not go around the vehicles in the line.
*If your student arrives after the 8:10 bell, you must come accompany your student to the front doors of the school to sign your student in. Please ring the door bell and someone in the office will come out to have you sign your student(s) in.
*If your student needs to leave school before the end of the day, please email your student's teacher. When picking them up, please come to the front doors and someone in the office will come to you to sign him/her out. Your child will be called to the office when you arrive.

Physical & Immunizations - All Kindergarten students are required to have a physical exam and an up-to-date immunization record on file by the first day of school. This is a state requirement for school attendance for both face-to-face and remote learners.
Medication - All medications, including cough drops and over-the-counter products, require a Medication Authorization Form to be completed and signed by a student’s healthcare provider and their parent/guardian.
