that Holy Name supports High School CYO Teams? If you have an interest in playing Volleyball or Basketball at a high school level, then you can sign up to play on Holy Name's CYO team. Watch the newsletters for more information.
Note from CYO: Due to the growing popularity of Boys High School Volleyball in the State of Indiana, beginning in September, 2019 the CYO will host a Boys High School Volleyball league in conjunction with the current CYO Girls High School Volleyball League.
Please know that the CYO guidelines must be followed. All participants must play for their parish of registration.

CYO Sports Registration
CYO Sports Registration:
CYO and the Holy Name Athletic Commission requires all students who play sports to have a physical on file. A student without a valid physical will not be allowed to practice or play games until it has been turned into the Athletic Commission. For all fall and winter 2019 and spring 2020 sports the student must have a physical dated after April 1, 2019. Sports physicals will be valid for an one year of sports from date of physical.
The CYO also requires that each student and parent read and sign concussion and sudden cardiac arrest forms each year. These also must be on file with the Athletic Commission.
The following links can be used to find the physical form, concussion/sudden cardiac arrest paperwork and our physical policy. These will be collected at the first team meeting of each sport where applicable.
