Important K-2 Hybrid Reminders

Monday, January 25, 2021

Don't forget to complete your student's Health Assessment form!

A daily health assessment (also called “attestation”) is required for all students before entering a school building for face-to-face learning.

This week, families will need to complete a printed copy of the Health Assessment form for each student attending school in-person.

Before sending your child to school, we ask parents to:

  • Take their child’s temperature

  • Complete the printed Health Assessment form (click below to download).

  • Send the form to school with your student.

Students will turn-in the form upon arrival at school and have their temperature taken again.

Students without a signed Health Assessment card must complete an in-person health assessment given by a staff member prior to entering school.

Forms were mailed to all K-2 Hybrid Learning pathway students. If you did not receive the form, click below to download.

Reminder! Students must must wear a cloth face covering or disposable mask while while at school and on the bus.

Gaiters, bandanas and face shields are not permitted.

Please visit for mask information and resources to help you prepare your student for wearing masks at school!

Students encouraged to bring refillable water bottles from home

Drinking fountains are turned off in our schools but water bottle filling stations will be available. Students are encouraged to bring refillable water bottles from home.

If your student does not have access to a reusable water bottle, please contact your child's school.
