· Students FULL first and last name
· Grade of student
· Parent/Guardian phone number
· Time of dismissal (not period) along with reason for dismissal
· Parent/Guardian Signature

After that time, you will need to come and sign your student out, even if your student is a driver with a car on campus. No student may be signed out after 2:15. If your student leaves without signing out properly, it will be unexcused.
If your student is ill at school, they will need to be dismissed via the clinic. The attendance office cannot dismiss students over the phone.
Absences:
If your student was absent from school, they need to bring in a signed parent note or doctor note to the Attendance Office upon their return to school. You may also send a doctor’s note via email to gwen.french@wcs.edu. All absences will be recorded as unexcused until a parent/doctor note with reason for absence is received. Please be aware that after 8 absences a medical note or administrative approval is required for an absence to be counted as excused. Please see board policy 6.200 for further information. The note should contain students Full name (First and Last), date(s) of absence, grade, and reason for absence.
Late Arrival:
If your student arrives late to school, they either need to be signed in by the parent/guardian or they will need to bring in a signed parent or doctor note explaining the reason for the tardy. If they do not have a note with them when they arrive, they will receive a detention.
Absence Call Out:
If your student is marked absent or late in the first few periods of the day, you will receive the daily attendance call out. This is for ANY absence reason, excused or unexcused. Attendance can be verified in Skyward.
***Please note, your student will need to request the Driver’s Permit form at least 24 hours in advance. The form is good for 30 days. We do not Email or Fax these forms, they must be picked up at the school!***
