1st Posting – 6th Grade Physicals (and Dental Forms)
By state law, 6th grade physicals must be turned in by Thursday, October 15. Failure to do so by that date will result in exclusion from school until the physical is submitted. If you have any questions please email our health assistant, Anna Ghukasyan, at aghukasyan@dps109.org.
Dental forms are not due until May but you are encouraged to turn those in as well to complete your state-required forms for the year. Please see the district website for details about required paperwork and vaccinations, or call Ms. Ghukasyan with any questions or concerns.

2nd Posting – Lake County Attendance Week
We will again participate in Lake County “Attendance Week” from Sept. 12-16. Caruso has been recognized in the past as a top performer and we would like to be there again this year!
3rd Posting - Yearbook Ordering Information
This year we are working with Jostens for our school yearbook. The yearbook is a great memory for middle schoolers as a keepsake of what their life was like during their time enjoyed at Caruso.
You can order a yearbook directly at www.jostensyearbooks.com or through our direct link http://jostensyearbooks.com/?REF=A01749750 or by phone at 1-877-767-5217.
Ordering MUST be done online- we do not accept cash or checks for the yearbook. The yearbook is $25.00 if you order before September 30th. After that, the price will increase.
Please keep in mind that yearbook is NOT a part of any fees you already paid- it is separate and optional to purchase. Don't let the memories fade away, order your yearbook today!

Also, you can make changes to your contact information at any time during the school year and those changes are automatically updated in the Caller! Once again, paper copies of The Caller may be printed from your Carusopto.org account but will not be distributed by the PTO.This is what we need you to do:

2. If you are a returning user of carusopto.org, login and skip to #4. If you are new to Caruso or did not have a carusopto.org account last year, you need to click on create your account (located under WELCOME NEW VISITORS on the left side of the home page).
3. After entering your information and clicking continue, select change (next to my profile) to add more parent information. *If you do not want to be in the Caruso Caller (directory), click the appropriate button at this time.
4. You must add your student's information to be listed in the Caruso Caller. To add student(s), select My Info at the top of the page and select My Student. Then select add new student and enter the information.
