Physical Night Tonight!

Save time...print your paperwork out ahead of time here and fill it out before you come!

Physical Night starts at 4:30. The cost of physicals will be $10 per athlete. We will have notaries on campus to complete the physical packets. Physical night is open to any student who may need a physical for any activity including club sports or summer camps that require a physical.

High School Students - 4:30-5:30pm

Middle School and Intermediate Students - 5:30pm

It's important for students to attend this night as there will not be another opportunity until the beginning of next school year. Also, please remember that physicals signed by nurses, nurse practitioners, and chiropractors will not be accepted by the AHSAA.

SRO for Meridianville Middle

The Madison County Sheriff's Department is funding three new SROs, including one for Meridianville Middle School. SROs act as both security resources within the schools as well as liaisons between the sheriff's department and the school system. This is great news for our cluster!

A Note From Nurse Jones

Dear Parents:

As we near the end of school, I would like to notify you of a few items.
Expired Immunization Records MUST be completed before the first day of school or your child will be sent home and removed from the school rolls. You will receive a notice this month if your child has an Expired record. Please do not wait until the day before school starts to have this completed. I would suggest making an appointment soon after you receive the notice to have this corrected.
If your child has meds in the clinic, don’t forget to pick them up before school is out. All medications must be discarded if they are not picked up by school's end.
We start all paperwork over at the first of every school year. That includes medication forms, Emergency Medication Plans, and Medical History forms.
• Last day of school for the students is May 21, 2015. My last day is May 22, 2015.

I hope that you and your child have a safe summer.

Jenni Jones, RN
