2020-2021 Immunizations
Typically we would send this information with report cards at the end of the school year. Since report cards are only accessible through Skyward, we want to ensure all families are also aware of new immunization requirements for the 2020-2021 school year. Meeting these requirements before the start of next school year is critical and we appreciate your partnership with accomplishing this task. Please review the information below. Thank you

Parents of incoming 9th, 10th and 11th Grade Students
Please note: New state required immunizations for 2020-2021 school year
These state mandated immunization requirements must be met before August 19th, 2020, which is the first day of school. If your child was fully immunized in 19-20, they only need the immunizations listed below for the 20-21 school year.
Two (2) Hep A (a series of two vaccines separated by 6 months)
We suggest you make appointments now to get the needed immunizations before next school year. The local health departments and area physicians are extremely busy in the spring and summer, as well as at the beginning of the school year.
If your child has already received one or both doses, please send in documentation to the school health office. You can also email or send a picture of the immunization record to sdikos@phm.k12.in.us
*Note: Hepatitis A- the minimum interval between 1st and 2nd dose is 6 calendar months. 2 doses are required for all grades pre-K-12. This is a new state requirement for 2020-2021.

Parents of 12 Grade Students

For senior year 2020-2021 your student will be required to have:
A booster of the meningococcal vaccine (MCV4)
2 Hepatitis A vaccines. (a series of two vaccines separated by 6 months)
These are requirements by the Indiana State Department of Health for your student to remain in school! It is also recommended that your student have the Meningococcal B vaccines and the HPV vaccines.
Please talk to your health care provider to schedule an appointment NOW! In order to not interrupt the start of your student’s senior year, we are encouraging you to get this taken care of as soon as possible. If your child has already received one or both vaccines, please send in documentation to the school health office. You can also email or send a picture of the immunization record to sdikos@phm.k12.in.us This would then be one thing to mark off of your list before next school year!
