UPDATES and Additional Information for Registration Days
Emergency forms and Family Income Information Forms (Lunch Forms) are now available to download on our website at the bottom of the page at this link: Pre-Registration Forms
If preferred, families can download and complete these 2 forms prior to attending a registration session. All other forms including medical, media consent, and others are now bound together in one hard copy booklet which will be given to families at the registration sessions.
- Child’s Birth Certificate/I.N.S Cards
- Proof of address: resides within the attendance area served by the Chicago Public School
- Illinois/State driver’s license and or Government issued I.D. /I.D with current home address
- Utility bills (combination of two- electric, gas or water)
- Lease, Mortgage or Condo assessment agreement/note
- All documents must show the name of the parent or legal guardian, the address and street name (No P.O Box).
Parents/guardians of returning students should bring proof of address as well any medical documentation that may be required.
