North Homes
Mahtomedi School District (ISD 832)
WELCOME BACK TO SCHOOL MAHTOMEDI FAMILIES!
Welcome to the 2020-2021 school year. As you all know, this is unlike any other school year we have had before. With the changes in today’s world, we understand how difficult these times may be for some of you. As the partnering mental health agency with your child’s school district, we would like to be available to support children and families more than ever. With that being said, we have made some adjustments in how we will be delivering mental health services.
As an attempt to stay better connected with our school communities, we will be sending home a monthly newsletter to all students, sharing relevant information and helpful tips relating to mental health. The newsletters will always include our contact information in case you and your family would like to talk to someone about initiating mental health supports.
Secondly, mental health services are not limited to just students. Adult and family supports are also available as we recognize there may be an increased need for these services. Additionally, if your child has opted into the full distance learning option, mental health supports are still available. For more information on services of any kind, please feel free to reach out to inquire about options so we can best meet the individual needs of your family.
How North Homes will be providing services:
In school - mental health services would be provided in school during the school day.
In home/community based- mental health services would be provided in the home or in a community location such as a library, youth center, park, etc.
Telehealth- mental health services would be provided using telepresence, where the provider and the recipient(s) would be at different locations meeting over a designated platform such as Zoom, Google Meets, WebEx, etc.
We look forward to servicing you and your families this upcoming school year!

Referrals & School Based Service
School-based mental health services strengthen students and their families through prevention and intervention within a school setting. For eligible students, a mental health professional meets with the student and family to formulate goals in conjunction with feedback from school professionals. Students may receive individual, family, and group psychotherapy directed towards changing and reducing symptoms of a mental health condition aimed at improving functioning at home, school, and in the community.
We differ from other behavioral support staff/programs in your building in the following ways:
*The students we work with need to have a current diagnostic assessment (which we will complete) in order to assess whether or not our services are medically necessary.
*The service we provide is structured from an individualized treatment plan, which we review with the student and legal guardian every 90 days. The work we do with students is always driven by his/her mental health diagnosis.
*A payer source is required for our services which ends up being the student's health insurance.
*We cannot provide support to students who are not enrolled in our services. In order to provide support we need the student's legal guardian to complete our intake paperwork giving us consent to provide treatment.
Who can make a referral?
- Community Worker (CPS, Social Worker, Psychologist)
- Parent - Self (18+)
- School Staff (Team Based Process, 1-2 Referring Agents per School)
**We do ask that all resources have been exhausted within the school system and you have consulted with your child's school guidance counselor regarding less intensive intervention prior to a parent referral being made.
**Our referral is linked below. If you are looking to make a referral please pass along referrals for Wildwood and OHA to Lindsay and Middle School/High School referrals to Sara. Contact information is listed above in bios.
