NEW this year, District 15 has launched online registration. If you are able, please complete the online registration process prior to attending registration events.
Click here to register your incoming kindergartener or new student for the 2019-20 school year.
Upon completion of online registration, parents must show the following documentation in-person to fully complete the registration process:
- Health forms
- Proof of residency
- Birth certificate
- Student transfer forms (if child is transferring from another school)
Turn in your residency, health and birth certificate forms during registration events beginning this week. Parents having difficulty completing online registration on their own will have access to computers and assistance during these events as well.

Please review our registration information website to learn more about registration and the required forms.
Spanish Dual Language
Information regarding two-way immersion Spanish Dual Language (SDL) kindergarten will be available for parents during registration events. An SDL informational night for interested parents will take place at 6:30 p.m. Wednesday, February 27 at Jane Addams Elementary School. Letters of commitment are due in March.
Download D15 mobile app to receive emergency notifications
When school is closed due to weather or another emergency, a push notification will be sent to anyone with the mobile app downloaded. This allows anyone not automatically enrolled in our mass notifications system to be notified of a closure without having to check the district's website or local media stations. Please note, parents and staff members (not including substitutes and contracted employees) are automatically enrolled to receive email, text, and phone call alerts with emergency notifications.
Additionally, the app provides quick access to staff directories, student information, lunch menus and more. Select your language of preference and receive notifications and information in the language of your choice.
To download the app, visit Google Play or the Apple Store and search for "CCSD15."
