A Message From The Principal

Dear Henry Hawk Families,

We hope everyone enjoyed the nice and cool weekend. Thank you to our families and the Henry community for supporting our Henry Community Garden last week with a clean up day.

We want to inform you that Chicago Public Schools was recently made aware of a data security incident involving one of our vendors that impacted staff and student information between 2015 and 2019. An email from CPS was sent to families over the weekend. If you do not receive an email or a letter from CPS, you can call the toll-free hotline at 833-909-4007 to verify if you were impacted by the breach and are therefore eligible for free credit monitoring and identity theft protection. Families will need to provide a student name and either an address, date of birth or student ID number. If you do not have these pieces of information when you call the hotline, the credit monitoring agency will escalate the issue to CPS so CPS can verify if you are eligible for the service.

This year we will again be hosting summer school as a Bridge+ site. We will also be holding other summer school opportunities. Teachers will reach out to you if your child is eligible to attend summer school.

Upcoming Dates:

May 24th: Middle School MSI Field Trip

May 30th: Memorial Day (No School)

June 2nd: 8th Grade Luncheon

June 9th: 8th Grade Trip

June 13th: 8th Grade Graduation 5:00 p.m.

June 14th: Field Day and Last Day of School

July 6th: Summer School Classes Begin

Please remember that if your child tests positive for COVID-19, please complete the self report on the following link: https://chicagopsprod.service-now.com/health?id=cps_self_report

As always, we will continue working to provide your children with a safe, healthy learning environment where they can reach their full potential.

Mr. Gutierrez, Ms. Reynolds, and Ms. Abraham

Medical Requirements SY23

Students with medications at school: Parents with students who have medications in the nurse's office, please do not forget to pick them up on the last day. Students with inhalers can have them sent home in their back packs however all other medications must be picked up by a parent or responsible adult. We do dispose of medications left behind.

5th grade parents: Next year your student is required to have an updated certificate of health (or physical) and Meningococcal and TDap vaccinations. Both sides are needed. If your child is 11 years of age and you have updated paperwork and immunizations, it can be sent to the office now rather than the start of next year. It can be turned in to the office or by contacting your doctor's office and having it faxed to 773-534-5042. If not, please schedule an appointment for your future sixth grader over the summer!

8th grade parents: An updated certificate of health (or physical) is a requirement for high school entrance. Both sides are needed. If your child has had an annual visit to their doctor since September 2021, you can send it to school now to meet the requirement early by sending it to school or contacting your doctor's office and having it faxed to 773-534-5042. If not, please schedule an appointment for your soon-to-be high schooler over the summer!

(It is important to note that the priority rules from CPS' Office of Early Childhood have changed this year.
