Nurse Notes
All medication that is currently at the school needs to be picked up on or before the last day of school, May 31, 2019. We will be disposing of any medication left at the school after that date. If you are unable to pick up the medication by end of the school day on May 31, please call the office to make arrangements.
- As a reminder for next school year, any over the counter and prescription medications to be taken during school hours must be brought to school by a parent in a prescription container with the name of the medication, the doctor, the amount to be given, and the time of day to be taken. Our District Medication Authorization Form must be completed before any medication can be given at school.
- If your child/children have a medical diagnosis, such as Asthma, Seizures, Food Allergies, etc… an Action Plan for that condition needs to also be completed. These documents can be found on our School website under “Parents” > Additional Forms
- Over the counter medications (i.e.: Tylenol, Ibuprofen, Claritin, eye drops, ointments, cough drops) also need a Medication Authorization form filled out in order to be administered at school. Please be aware that these items need to be brought in from home as the school does not have them on hand.
