ONE MORE DAY - Rolling Meadows Schools Food Drive
This year the PTAs at the five District 15 schools in Rolling Meadows will be participating in Food Drive from Feb.17th - Feb. 26th. Each school will be collecting food and other household essentials to be donated to one one of the local food pantries to help our local families in need. These dates were selected to align with one of the material pick dates to allow our Virtual Students and families to participate as well. For every item brought in, you child will be entered to win a chance to throw a pie in the Principals face! See the attached flyer from the PTA with the items needed. Our goal is to collect more than the other 4 schools!

Kindergarten Registration for 2021-22 School Year
Kindergarten Registration for the 2021-22 school year is now open for Online Registration. All students who turn five-years-old on or before September 1st are eligible to attend Kindergarten in D15 next year.
For ALL students who attend CLA, when you register online, you will need to select “EXISTING D15 Families.” If your child has older siblings in elementary or junior high, you will also be asked to register them as well at the same time. The Infinite Campus site will require you to sign in with the username and password you previously created.
ALL Kindergarten students will be required to go into their home school and provide them copies of their child’s required medical forms, three proofs of residency, and birth certificate. The CLA Office will not be able to pass on the copies provided when you enrolled your child at CLA.
If you would like further information on the registration process, please refer to the CCSD 15 Registration Page on the website. https://www.ccsd15.net/Page/9066
If you are not sure which elementary school your child will attend, type in your address to this website - https://www.locatemyschool.com/map/ccsd15#
Students with an IEP - Every student with an IEP at CLA will have a transition meeting before the end of the school year to discuss your child’s kindergarten placement, services, and supports. For some students, they may be recommended to attend an elementary school other than their home school with a specific program. During this registration process, please submit all required documentation to your home school. If you have any questions, please contact the CLA Office.
year will be required to fill out a new Home Language Survey form. These forms have been sent home with your child (virtual families will get the form at the next material pick up date). We ask that you compete and return this form as soon as possible. For any students who have a second language spoken at home, we will be scheduling a time to complete the language assessment while they are at school prior to the end of the year. This assessment will help determine if your child qualifies for Second Language supports and services in Kindergarten.
If you have any questions regarding the registration process, please do not hesitate to contact the CLA or your Home School Elementary Office for support.
5-Essentials Parent Survey
Every year the State of Illinois requires Parents, Staff, and Students to complete the 5Essentials Survey. Each school needs to have a specific number of families to complete the survey. We need your help to reach our goal! Please take a few minutes to complete the Parent 5Essentials Survey for CLA. If you have children at other D15 schools, you can complete the survey once for each school. Thank you for your input and help!

Student Health Certification
As students return to school, parents are required to certify that their child is symptom and exposure free each day. Before you send your student to school, a parent must sign the student’s Health Certification lanyard for that day. By signing the lanyard every day you are certifying that the answers to the questions and symptoms below are all ‘no’.
- Fever of 100.4° or higher
- New onset of moderate to severe headache
- Cough
- Sore Throat
- Vomiting
- Diarrhea
- Shortness of breath
- Abdominal pain from unknown cause
- New congestion/ runny nose
- New loss of sense of taste or smell
- Nausea
- Fatigue from unknown cause
- Muscle or body aches
Have you been told by a contact tracer that your child has been in close contact with a COVID-19 positive person in the last 14 days?
At the end of each week a new card will be placed in your child’s lanyard for the following week. Please note that the card must be dated and signed each day prior to school.
If a child comes to school and they do not have a lanyard or their lanyard is not signed, your child will be assessed by the school nurse, and a reminder note will be sent home, or phone call will be made. If the student shows any symptoms of COVID, they will be brought to the Protective Health Space and will need to be picked up from school immediately.
Additionally, if a student develops any symptoms of COVID throughout the school day (see list above), they will be brought to the Protective Health Space and will need to be picked from school immediately.
If your child was exposed to anyone who tested positive, please call our school nurse to discuss the necessary quarantine timelines.
