Purchase a School Supply Kit Through 5/25

School Supply Kits are on sale now through May 25! This is such an easy way to shop for your student's school supplies. Supplies will be waiting in your child's classroom at Open House in August. Visit https://www.educationalproducts.com/shoppacks/ and use our school code NEW468.

If you do not want to purchase a kit, supply lists are currently posted on our website.

Student Items

Families - if your child was absent, traveling, etc. at the end of the school year, please let your child's teacher know if you need to pick up personal items (yearbooks, school supplies, etc.). You may pick up items beginning Tuesday, May 30. The office is open Mon.-Thurs., 8:30AM-2PM, closed 12-1PM for lunch.

**Important Information**

Parents of Students with Medical Conditions and Medications please see below in preparation for the 2023-24 School Year:

If your child has a medical condition that requires possible intervention, medication, or the need to self-carry emergency medication while at school please submit the appropriate action plan forms every year to the school nurse. The forms must be signed by your child's doctor and are effective for one school year.

Please click here to print the packet of Action Plan Forms for medical conditions including the following:

  • Food/Insect Allergy Action Plan
  • Asthma Action Plan
  • Seizure Action Plan
  • Diabetes Management Plan
  • Individual Healthcare Plan for conditions not listed

** Medication Reminders**

  • For any medications (prescription or over the counter) please complete the Request for Medication Administration Form
  • Prescription meds will require a doctor's signature if being administer for greater than 10 days.
  • Medications may not be transported to or from school by students, it must be a parent or guardian.
  • ALL Non-emergency medications must be stored in the clinic. This includes over the counter medications such as Advil, Tylenol, Cough Drops, etc.
