Note From Principal
Dear F.AVE families,
Welcome to the 2021-2022 school year at Fifth Avenue Upper Elementary! We are thrilled to welcome all new and returning students on Tuesday, August 3rd. We will work tirelessly to provide a great education for your students—an education that not only prepares them for the future but that allows them to experience their greatest selves. We strive for F.AVE to be more than a school, but a family of closely connected lifelong learners.
We are returning to a full day of in-person learning for our students. We will continue to have some of the mitigation strategies from last school year in place. Those are outlined in our CSD COVID Mitigation Strategies document. We are excited for the learning and growth students will experience this year!

New Third Grade students:
We created presentations for rising third grade students and parents spring 2020. They are still great resources for kids and families. We are excited about our incoming 3rd grade Falcons and want you to hear more from past third graders about being a learner at F.AVE.

Meet and Greet:
Meet and Greet will be held virtually. They will be held on Friday, July 30th. Your teacher will reach out to you on July 27th with times that she will meet in small groups with families.
You will find out your child’s teacher as usual via the Infinite Campus portal. These will be available on July 27th. At this time, we cannot honor any class change requests, except twin placements. Our rosters have been created to ensure balance on various levels. If you do not know how to access the parent portal, please contact Malik James, F.AVE’s Administrative Assistant at firstname.lastname@example.org.
School Tours: For rising 3rd and 4th graders who did not attend in-person are happening this week! Today is your last chance to sign up. We want to be sure we have enough people to give small group tours each day. Sign up here!
Instructional Books/Materials and School Supplies:
Our School supply list is linked here for all grade levels. School supplies are suggestions of tools that could be used at home and/or when we return to school. Many of you took advantage of ordering supplies through our PTO. Those supplies will be delivered to your home.
CSD schools are all transitioning to Thursday as the communication day. We will send school-wide communication to families through our Falcon Flyer each Thursday. In addition to this, teachers will send weekly newsletters that give specific classroom level information. The F.AVE’s website is also a great source of information that’s regularly updated.
Parent Portal/Infinite Campus:
Families new to CSD will receive an email from Infinite Campus with a prompt to set up a parent portal account. The parent portal allows parents to view their child’s attendance records, assessment records, lunch balance, and an online lunch payment option. Parents can customize their contact information to ensure they receive weekly newsletters, and emergency school and CSD robo-calls. This is also how classroom assignments will be shared prior to the start of school. F.AVE’s PTO Website:
This is another great source of information about the school, including events, membership, and how to connect with other parents. The PTO is focused on supporting the instructional program of the school and building community. Learn more at F.AVE’s PTO Website.
This handbook is a one stop shop to answer most questions you may have about our school. If the question isn’t answered, use the last page of the handbook to locate a direct contact for your question. If you don’t see a contact there, just email the Adminstrative Assistant, Malik James and she can assist you.
School Leadership Team:
The Fifth Avenue Upper Elementary School Leadership Team (SLT) is an elected committee consisting of F.AVE parents/guardians, staff members, a community member, and a district representative. The purpose of the SLT is to make decisions that support the continuous improvement of the school in accordance with the provisions of the District Charter. This year we will have 2 vacancies on the SLT and will have the SLT election in late August. If you are interested in filling one of the vacancies, email a short bio of yourself and why you are interested in serving to firstname.lastname@example.org. If you have any questions regarding the SLT, feel free to contact any of the representatives below.
Annual Data Update
Families- we have moved the typical beginning of the school year household data update, medical forms, opt-out forms, and handbook sign-offs to the Infinite Campus Parent Portal. On Monday, July 19, parents will receive an email with a link to access the update in the parent portal.
It is important for parents or guardians to log into the parent portal and complete these forms before the start of school. Please note, only parents and guardians in the primary household will have access to fill out the information for the annual data update. Please review the 2021-2022 K-12 District Handbook, Code of Conduct and Restorative Practices Handbook, and Consent for Rapid Covid testing prior to completing the update.
Directions for accessing the data update
Log in to the parent portal
Once you are logged, click more on the bottom left.
Then, click Annual Data Update
Move through the questionnaire reviewing and updating all the required fields marked with a red asterisk.
Once complete click save/continue.
Follow the same process for each of your children.
Click Submit to finalize the update.

Pickup Patrol App
F.AVE will use PickUp Patrol (PUP) is a secure app that eliminates the need for you to send notes, write emails, or call the school to change your student’s dismissal plans. From a smartphone or a computer, you can make plan changes anytime, anywhere—days, weeks, or even months in advance. It's simple and convenient to use—but most importantly, it helps your school manage student arrival and departure information accurately and efficiently to help keep your students safe.
All families will need to update default plans because this is a new school year! Sometimes plans change.
REGISTRATION & GETTING THE APP
How do I register? We will send you a registration email. Use the link in that email to set up your password. We will be sending out emails to families by next week. If you haven't received yours by Wednesday July 28th, check your spam/junk folder and add email@example.com to your contact list. If it’s not there, contact Jessica Wentworth to confirm that we have sent out these emails and that they have your correct email address on file.
