The contact information can be utilized to schedule play dates, birthday parties or other social events outside of school. By submitting your information on this form, you are providing Virginia Lake with permission to share your information with the families of students in your child’s classroom. Please complete this form if you would like to be included in the social directory. All of the families in the classroom will receive the information. It will be emailed by the classroom teacher. Please complete this form for every child in your family. Please complete this form by Friday, September 16th.

Mental Health Awareness
Community Consolidated School District 15 is committed to supporting every student’s academic growth as well as their social, emotional and behavioral growth. In our continuing efforts to support the well-being of our students, District 15 is implementing a comprehensive mental health screening process this school year to identify students who may benefit from additional support. As part of this process, District 15 is utilizing the Social, Academic, Emotional, Behaviors Risk Screener (mySAEBRS) this school year. The screener, mySAEBRS, is a brief rating scale which will be completed by all students in grades 5-8th. Student screeners and presentations, for 6th-8th grade, will be taking place during the months of October, November and December. Fifth grade screening and presentations will take place in late winter (January/February). Another component of the comprehensive mental health screening process is a Mental Health Awareness program through Erika’s Lighthouse. This program provides school staff the opportunity to teach students about mental health and teach them how to seek help if they are concerned about themselves or a friend. Students participating in this mental health awareness program will be reminded that hope and help is available in a variety of places including their own school. The student program consists of a powerpoint presentation that includes a mental health awareness video, a group discussion and ends with a student exit form in which students can choose to ask to talk with a school mental health professional. Self-referred students and students referred by peers will be interviewed by a school mental health professional, and parents will be notified. On Thursday, October 13 at 7 PM, all parents are invited to an informational presentation which will include a showing of a Mental Health Awareness video. Following the presentation, questions will be taken and there will be links provided to information on mental health and a list of local mental health agencies. All materials from the evening will also be available in Spanish. We are enclosing a copy of community resources and good mental health tips so that you have information and resources about depression and its related risks. If you do NOT wish your child to participate in the Social, Academic, Emotional, Behaviors Risk Screener (mySAEBRS) screening process and/or the Mental Health Awareness program in school, please contact your building principal. If we do not hear from you, we will assume your child has permission to participate in this program.
If you would like to opt out of the program, please print the attached document and return it to Virginia Lake. You may also request a printed copy.
