Reminders
The weather is starting to change! Please make sure your child is dressed for the weather each day. If the temperature (including wind chill) is above 0 degrees, we will be outside for at least 20 minutes. We want our students to get outside to run around and play, but it will be important for them to dress in warm coats, hats, gloves, boots and snow pants! Thank you!
Please make sure your children have masks that fit properly. If a mask is too big, it does not cover the nose. When possible, send your child with an additional mask as students have had some wet or torn masks throughout the day. Please wash cloth masks regularly.

Writing in 2nd Grade
6th Grade Math Fun
Students in Mrs. De Anda’s 6th grade math class worked on calculating constant speed. They timed themselves traveling along a 10 meter path and calculated how many meters they traveled per second using double number lines!
Book Talks
Mrs. Jauch’s second grade class learned how to give a book talk to inspire reading!
District 15 Updates
Optional Vaccination Clinic - Transportation can be requested!
District 15 is sharing information regarding a second opportunity for families to have their children ages 5 -11 years old vaccinated against COVID-19. In addition to 5-11 vaccinations, IDPH will also have adult vaccinations and boosters appointments available.
The Illinois Department of Public Health (IDPH) has partnered with District 15 for this second vaccination opportunity. Parents/guardians MUST accompany their child(ren) to the clinic.

Dates:
Dose 1: Saturday, December 18, 2021
Dose 2: Saturday, January 8, 2022
Appointment Times:
10 am - 3 pm

Conyers Learning Academy is located at 2800 Central Road Rolling Meadows, IL
All vaccination appointments will be held in the multi-purpose room/cafeteria. Parking will be available at CLA and guests are asked to enter the building using Entrance 15.
Additional Information:
No insurance is needed.
District 15 will be offering transportation to/from the vaccination clinic from various locations in the area. If you would be interested in transportation, please fill out this form. Transportation will be provided based on need/responses to this survey.
Vaccine recipients should wear vaccine appropriate clothes (short sleeves) to expedite the process.
We encourage you to ask your child to pick out their favorite stuffed animal to bring with them; let them know that their animal friend will be able to get a shot, too!
Documentation/Forms to Bring to the Appointment:
COVID Vaccine Consent Form — parent/guardian must sign the form for their children under age 18.
Please bring a copy of parent/guardian ID.
Submitting Your Child’s Completed Vaccination Card
Once your child has received both doses of the vaccine, we ask that you please submit a copy of it to your school nurse. Fully vaccinated individuals are not identified as close contacts, and therefore, not excluded from school if there is a positive COVID case (as long as they remain asymptomatic). This documentation is very important during our contact tracing efforts. Families will continue to be notified if a student is identified as a close contact so they are aware to monitor for potential symptoms.

Mini-Grant Fund-a-Thon
The one-five Foundation is once again offering an annual mini-grant program for staff members of Community Consolidated School District 15. These proposed projects support a wide range of opportunities for the students of the District with a focus on programs or technology.
This year, 13 grant applications have been preliminarily approved by the Foundation for funding. While the one-five Foundation Teacher Mini-Grant Committee has preliminarily approved these projects, additional funds are needed to make them a reality. These amazing projects could have an impact on thousands of students across all grade levels in 12 different school locations throughout the district.
You can help! Please consider making a donation to the one-five Foundation Teacher Mini-Grant program to help our teachers help our children. Project details can be found on the one-five website along with options for online donations.

This communication tool allows parents and students to quick access to important school and district information, including Campus Parent, the D15 Bullying Tip Line, a quick link to MyStop (bus stop location and pick-up times), lunch menus, schedules, parent/student handbook, directory information, school news, and more.
To see integrated student information on the app, click the "student information" button and log-in using your email address and your Campus Parent password. If you are unsure of your password, please visit Infinite Campus and click "forgot password." If you are still having trouble, please contact your school's main office for assistance.
To make sure you aren’t missing any notices, make sure you turn notifications ON for the mobile app.
