A Note from the Nurse
If your child needs an immunization to be up to date with state standards, I have been contacting you by letter or phone. I will work with you to schedule appointments, and MCHD has walk in clinics and appointments for immunizations each week (MCHD South District office 505 E National Ave; 317-221-5700), but we are approaching a deadline for compliance in October. Please schedule your appointments and bring me the updated shot record after obtained or have the record faxed to 317-862-7255. We do not want any student to miss instruction time due to missing immunizations.
NOTICE OF THIRD AND FIFTH GRADE VISION SCREENING
On Monday September 26th, the Marion County Health Dept. will be at Adams Elementary to perform vision screening. All children in the third grades, fifth grades, students who are new to Marion County, and children from other grades suspected of having a vision problem will be screened this day. Please mark your calendars for this important day. If your child already wears glasses, please be sure that they are wearing them for the screening.
In School Dentist Visit Date: Friday 9/9/2016
This year, Franklin Township has joined the Indiana Dental Outreach to offer in-school dental care. This is an OPTIONAL service being provided to our families in need of dental care. If you would like for your child to be seen by the dentist contact me in the clinic to request a form to fill out. Your child WILL NOT be seen by the dentist if a form is not returned to school. The form is attached for you to complete. It should be returned to the clinic as soon as possible if you would like your child to be seen on 9/9/2016. It can be scanned and emailed to me at email@example.com or brought to clinic by your student. Please call the clinic if any questions, 317-860-4289.

Visitors to our building can now pre-register to come into the building for special programs, volunteering, or lunch. By doing this, the sign in process once in the building should take about 15 seconds.
