FInals Permission Form & Letter Please Return by May 24th
Technology Return
1. Your child must turn in their device and power cord after their exams. Please know that the device will be inspected at this time for damages and repair needs.
2. You must attach the completed summer collection form to the top of your device with masking tape (the form and tape will be available in the Library beginning May 20.)
3. Your device must be fully charged when you turn it in.
4. Your device and power cord must be labeled with your name and graduation year. (masking tape and pens will be available in the library during collection if your cord is not labeled)
5. If you do not turn in your device prior to leaving school on the last day, the replacement cost ($225.00) will be charged to your account. If you do not turn in the power cord the fee ($20.00- 25.00 depending on the model), will be charged to your account. Please be aware that if the power cord is not turned in a new one will be ordered so it is essential that students bring it with them at this time!
6. Repairs made to your device over the summer will be first applied to your insurance (if it has not been used up for the year) and secondly applied to your account.
2019-2020 School Transportation
ATTENTION BUCKEYE PARENTS:
If your student rides a bus to or from school and any of the following changes apply to your student, you will need to complete a new Transportation Request Form for the 2019 -2020 school year:
Your Student is rolling up from Preschool or 6th Grade.
You have moved to a new residence (You must first contact your student’s school or Jill Besida at the Board Office and provide a new proof of residency).
You have made changes to your babysitter or daycare location (Requests must be for five days per week to the same address).
The 2019 - 2020 Transportation Request Form is available on the Buckeye website under transportation. A paper form can be provided upon request.
ALL TRANSPORTATION FORMS ARE DUE BACK TO TRANSPORTATION BY JULY 19, 2019 TO GUARANTEE TRANSPORTATION AT THE BEGINNING OF THE 2019-2020 SCHOOL YEAR.
Information regarding bus routes for the upcoming school year will not be available until the 2nd week of August. At that time transportation information letters will be mailed to all registered bus students for the 2019 - 2020 school year.
Any changes requested after the letters go out will not be made until the 2nd week of the school year. Parents will be responsible for transportation until then.
