2021-2022 REQUIRED ATHLETIC FORMS NEEDED FOR ALL ATHLETES

ALL forms MUST BE turned in BEFORE an athlete can participate in mandatory tryouts, practices and/or competitions – (MHSAA Physical Form-dated after 4/15/2021, the Athlete/Parent Responsibility Form & the Henry Ford Allegiance Consent For Treatment Form – forms are attached below). Once an athlete’s paperwork is turned in & grades are checked, they will receive a tryout/practice ticket. Without a ticket from the athletic office, they will NOT be able to participate. ALL HS Fall Sports begin mandatory tryouts/practices on Mon. August 9th.

NW High School Fall Try Out Information

Every athlete MUST have an MHSAA Physical on file in the HS Athletic Office along with the Athlete/Parent Responsibility Form & Henry Ford Consent for Treatment Form – NO EXCEPTIONS. Once all forms are turned in, EVERY athlete MUST have a tryout ticket from the athletic office to participate.


Mandatory Tryouts/Practices for High School Sports begin Monday, August 9, 2021. Below are the dates/times for each sport:


Monday, August 9

HS Boys/Girls Cross-Country – 9am and/or 6pm (you are required to attend at least one of the two practices – but you can attend both if you wish)

HS Girls Golf – 3:30pm at Hankerd Hills (weather permitting – bring your own clubs)

HS Football – 4:00pm – 9:00pm

9th/JV/Varsity Volleyball – 4:30pm-6:30pm (T-Shirt Colors – Freshman-White, Sophomores-Grey, Juniors-Red, Seniors-Black)

JV/Varsity Sideline Cheer – 5:30pm-7:00pm (Red or Black Cheer Shorts & a NW T-Shirts/Tank Top & Tennis Shoes)

JV/Varsity Boys’ Soccer – 5:00pm-7:00pm

Boys Tennis – 6:00pm-8:00pm



Tuesday, August 10

HS Boys/Girls Cross-Country – 9am and/or 6pm (you are required to attend at least one of the two practices – but you can attend both if you wish)

Varsity Volleyball – 11:00am-12:00pm ** this is an additional practice after the morning tryout time

HS Football – 4:00pm – 9:00pm

Boys Tennis – 10:00am – 12:00pm

** practice times for all other sports will be given to the athletes after Wednesday’s tryout/practice

Equestrian Teams

Northwest has a middle school and a high school equestrian team. These teams compete with their horses at meets which operate much like a track meet. The coach puts the riders in events as individuals, sometimes as teams of two, and the riders earn points for the school team depending on how they place.


The junior team consists of students 6th - 8th grade, who either live in our district or live in a school district that does not currently have a team.The senior team consists of students from our school district and are in 9th-12th grade. It is necessary to have a horse or the use of a horse to be a rider on the team. Perhaps you or someone you know has a horse that needs a kid to work with? That owner could consider loaning or leasing out the horse.

The junior team practices once a week late winter through spring, and then shows in spring. The senior team practices once a week all summer and shows in the fall. Senior team enjoys many of the same opportunities as other sports such as extended meets for Regionals and States, as well as earning their letter for this sport. Check out their trophies, in the display cases in both buildings!

2. Fill out the application info and Emergency Contact info on the CONTACT page. Click Next.

3. Select your choice for the optional Travel Protection Plan on the INSURANCE page. Click Next.

4. Review the optional excursions, rooming & travel plans on the CUSTOMIZATION page. Simply select any of the options you wish to have. Click Next.

5. Choose a payment plan and method that is most convenient for you on the PAYMENT page.

6. Review all of your info and tick the boxes to confirm all of the legal info on the CONFIRMATION page. Click “submit application” to complete enrollment.
