Parent Input Form SY 2021-2022
To help our staff and the administration prepare for the next school year we allow for parents to give input on how their child's experience has been this past year. If you have multiple children and would like to fill out the form we ask that you fill out one per child. Please understand we must consider many different factors in order to create balanced classes for the next grade, and we cannot consider requests for particular teachers (other than to consider sibling history or multiple siblings when possible). This form will be live until May 14th.

Kentucky has been approved by the USDA to provide Round 3 of P-EBT benefits to cover the period of October 2020 through May 2021. All BGISD students are eligible for this benefit of $6.82 per day. Students that are primarily enrolled in a school that is VIRTUAL will receive 20 days of benefits for the month, students enrolled in a school that is predominantly HYBRID will receive 12 days of benefits for the month. All students that are primarily enrolled in a school that is predominantly IN-PERSON are not eligible for any benefits for the month.
All benefits will automatically be provided to the student’s previous P-EBT card for August and September unless they currently receive SNAP, where the benefit will then be added to their current SNAP benefit card instead. Families can request a replacement card if they no longer have the previous card.
All questions should be directed to 1-855-306-8959, including reconsideration requests.
