Required Medical Forms
For students starting Kindergarten, Second, and Sixth grades, required medical forms can be found through this link. (Kindergarten students cannot start school without current physical and immunization records on file.) If parents would like to send any forms in for the upcoming school year, they have the option of sending them to the nurse at jsauck@d47.org.
Arrival / Dismissal Procedures
Arrival: Morning arrival starts at 8:30am and concludes at 8:50am. Students may be dropped off or arrive any time within that 20 minutes. Our buses typically arrive between 8:30-8:45am.
Once at school, students place their backpack in their class line and may enjoy recess with their peers. Supervision is provided by our staff members and our supervisors start at 8:30am. Each will be wearing a neon-colored vest at all times. Should there be inclement weather, students will come into the building and be supervised inside.
Dismissal: At dismissal (3:30pm), students will head to their appropriate area, whether it be meeting a parent/guardian in the front or back of the building to walk home, heading to the car pickup lane to be driven home or, lastly, to the auditorium to wait for their bus route to arrive. Husmann staff will be handling supervision duties after school as well. You will see staff in the pick-up lane by the front door while other staff supervisors are overseeing students who ride the bus in our auditorium. All bus riders will be grouped according to their bus route.
