EMS ISD partnering with Perrone Pharmacy

COVID-19 Vaccination Clinic for ages 5 and up

EMS ISD is partnering with Perrone Pharmacy to host an on-site COVID-19 vaccine clinic. The clinic will be sponsored and run by Perrone Pharmacy, located in Fort Worth since 1952. The Pfizer vaccine will be offered to individuals 5 years of age and older. COVID-19 vaccines are not required for school attendance.

First dose clinic was held on November 20,2021

Second Dose Clinic

Date: December 11, 2021

Location: Hollenstein Career and Technology Center (5501 Marine Creek Parkway)

Time: 9am-3pm

Booster Dose Available December 11, 2021

Both Moderna and Pfizer booster doses will be available at the clinic held on December 11th. Booster doses may be given 6 months after completing the initial vaccine series. Please bring your original immunization card with you if you are receiving a booster dose.

How to Sign Up

Individuals that attended the first clinic have already been given an appointment for the second clinic by Perrone Pharmacy, and there is no need to register a second time.

If you are registering for a booster dose you can register two ways:

Use this link by midnight, December 10, 2021: EMS ISD Pfizer COVID-19 Vaccine Clinic

Please select a time slot and put in the name and birth date of the individual being vaccinated, the parent's email, and parent's cell phone. Once you are done hit the "agree" button. You will receive a confirmation email.

Families that have multiple students please select a time slot for each individual receiving the vaccine. If time slots are not available back to back please still select the next available time and arrive to the clinic as a family unit. Siblings will be vaccinated together.

Or, if you prefer, please call the Perrone Pharmacy COVID Hotline 682-404-8796 and leave your name, your child's school and a telephone number to have the pharmacy call you back and schedule an appointment for the clinic.

Form 1 COVID-19 Consent and Release Form

This is the form giving consent for the COVID-19 Vaccine. All individuals receiving a vaccine must fill out this form. The form needs to be signed and dated the day the vaccine is received. Please print out the form and bring it with you. Individuals under 18 must have a parent or guardian signature.

Form 2 Minor Consent (under 18)

This form is giving permission to report to the Texas Immunization Registry. This form is needed for individuals under the age of 18. A parent or guardian signature is required.

If you are unable to print forms copies will be available on the day of the event.

On the Day of the Event

Please bring:

  • Completed forms
  • A form of ID from the parent
  • Insurance card if you have insurance
  • Your student's Social Security Number (you do not need to bring the card itself)
  • If you do not have insurance you will still be able to receive a vaccine at no cost
  • Original COVID 19 vaccination card

If you have not filled out the paperwork before the clinic, please arrive fifteen (15) minutes before your appointment to complete the forms. Blank forms will also be available at the clinic.
