Life Insurance Open Enrollment
September is Life Insurance Open Enrollment
Employees currently covered by the District's Life Insurance plans may purchase an additional $30,000 or $50,000 of Supplemental Life Insurance. (See rates below.)
If you are interested in applying for Supplemental coverage, please email Danielle Crawford at dcrawford@avongrove.org for an application packet before September 29, 2017.
Employees who do not wish to apply for supplemental insurance or employees who already have supplemental coverage do not need to do anything at this time.
Do You Need To Update Your Beneficiary?
Now is an excellent time to consider whether you need to update your Life Insurance Beneficiary or his/her contact information. If you would like to do so, click HERE for the Beneficiary Designation Form. Return your completed form to Danielle Crawford.
