About the Delta Academy Chair

My name is Conswalia Chambers-Banks and I am the committee chair for Delta Academy for the Knightdale-Wake Forest Chapter of Delta Sigma Theta Sorority, Inc. I am a school counselor for Zebulon Elementary School (WCPSS), but I also have experience at both the middle and high school levels.

I am very excited to work with your daughter(s) this year!

Participant Applications

In order for your daughter(s) to participate in Delta Academy, you must complete and submit both a Youth Initiatives Application and Consent Forms. Both of these documents are attached below.

I am asking that all applications and consent forms be turned in to me prior to or at the October Session.

Any participant without the required paperwork will not be allowed to participate (beginning November 2017) until documents are received.

Risk Management Approval

Parents are not required to stay for the sessions. However, if you would like to stay for sessions and/or participate in our college/university visits (November) and community service activities, you will need to be Risk Management Approved.

In order to become risk management approved, you will need to complete the attached application, pay a $25 fee for a complete background check and attend a training session.

Parent Communication

New this year, I will send a monthly newsletter regarding the upcoming month's session. After this month, the newsletter will likely be much shorter.

In addition to the newsletter, I will send a reminder email the Wednesday before the Saturday's session.

If you have questions for me, please don't hesitate to communicate with me via email, cell phone or text.
