SUMMER ADDRESS CHANGES

OFFICE HOURS START Monday, August 13th.

IF YOUR ADDRESS HAS CHANGED OVER THE SUMMER PLEASE STOP IN TO UPDATE YOUR ADDRESS!!

Change of address forms are on the SC web page or you can pick one up in the office.

Along with this form for in-district address you will need 2 proofs of residency.

Drivers license with updated address

consumers bill or lease/ mortgage paper

Medication Policy

Medication will be administered to children only if the Authorization to Administer Medication Form is on file in the Dieck Office. Medication must be in the original prescription container with he child's name and name of the doctor on it. The medication will be stored in a locked container and administered as prescribed by the physician. Administration will be recorded on a medication log sheet by the staff.

Please do not send medication in with your student. You have to bring it in and fill out the appropriate forms.

To Walk in the PARADE be at the Middle School tennis courts parking Lot by 4:00 p.m.

Attire: Dieck wear, Creek wear or Red and White Swartz Creek clothing and Comfortable shoes.

Candy: may be handed out but not "thrown".

Permission Slips: Must be turned in by Thursday, Sept. 19th for your student to attend.

Every student must be accompanied by an adult.

We hope you can join us!!!!
