Surveys To Complete for SWPBIS (School Wide Positive Behavior Intervention System) & SIP Goal 3

Our school is working on SWPBIS (School Wide Positive Behavior Intervention Systems) to provide clear expectations for students on appropriate in various locations in the buildings including, but not limited to cafeteria, hallways, classroom, lavatory, etc. As we begin this implementation, we are working to collect pre-implementation data. As a result, we are asking that Teachers complete these two surveys that will take approximately 20 minutes combined for both surveys. You may complete these during the 1 Hour Meeting, after school, or during conferences. All surveys need to be completed by Friday, December 4, 2015.

PBIS Assessment is a web-based application designed to assist in high-fidelity, sustained implementation of school-wide positive behavioral interventions and supports (SWPBIS). A major feature of SWPBIS is the commitment to ongoing assessment of implementation. PBIS Assessment provides surveys for teams to take as they examine their level of SWPBIS adoption and guides them through the process for improving implementation to benefit students, families, and the overall school culture.

Please complete each of the two surveys independently, following the links provided:

The School Safety Survey (SSS) is a multiple-response survey taken staff to help teams determine risk and protective factors for the school. The SSS summary identifies what training and support may be needed related to school safety and violence prevention in the school.

This is the link for the Safety Survey:

The Self-Assessment Survey (SAS) is an annual, multiple-response survey to help teams identify the staff perception of implementation status and improvement priority for school-wide, classroom, non-classroom, and individual student systems.

  • Mark Shuster
  • Lisa Childs
  • Jim Swiggard
  • Antonio Montes
  • Steve Burdan
  • Todd Van Horn
  • James Dargan

Goal 3 SIP School Improvement Plan .

Goal 3 in our SIP is focused on providing interventions and support for students. See below:

Develop an integrated SWPBIS K-12 model for Tiered Intervention Support (MTSS= SWPBIS +RTI)

Action Steps:

  • 1. Attend Tier I, Tier II and Tier III training and technical assistance by PaTTAn and MCIU
  • 2. Develop HS SWPBIS team.
  • 3. Develop a SWPBIS handbook.
  • 4. Establish baseline utilizing the SAS survey and discipline data to assist with the implementation of building based action plans.
  • 5. Create and show videos on Matrix Domain areas

DiLE Rubric Walkthroughs Begin December 1, 2015
