Dear Families,
Included in this letter is the Rock Eagle Permission form and information to help make this trip to Rock Eagle a fun and safe experience for all. Please read through all of this information carefully and make sure that everything on the checklist below is taken care of by the appropriate deadlines. Rock Eagle has a very informative website. You can find it at www.rockeagle4h.org.

Chaperones Needed- We Can't Do It Without You!
We will need parent chaperones. We can’t do this trip without you! If you are interested please complete this form. Chaperones will be accepted based on the need for additional support. We will not know the exact number of chaperones needed until after the forms and payment have been finalized. Once forms are turned in we will contact parents who completed the form in the order the request was received. Parent chaperones will be required to submit a background check with CSD and pay the $35 dollar fee.
Departure/Return
Students will need to arrive at 8:30 a.m. on the morning of their departure. Here is a packing list, please follow the packing rules of 1 carry-on/travel-size bag per student. The bag should be no larger than carry-on luggage.
Return:
Students will return to BHMS on their expected return day by 3:30.
Trip A students will be dismissed during regular dismissal time with school dismissal procedures.
- Trip B students will need to arrange transportation outside of the school bus as the return time is on early release day school buses will not run their normal route.
Medication and Health Restrictions
Staff will administer medications to your student. All medications, prescription and over-the-counter, require a signed medication authorization form. Prescription medications also require the medical provider’s signature. Medication Authorization form: here. 1 medication per form. Per CSD policy, students may ONLY self-carry Insulin, EpiPens/Auvi-Q, and Inhalers. Please fill out the Authorization for Student to Carry Medication form. The student, parent, and medical provider must sign. Please write on top of the form where the medication will be located (fanny pack, purse,etc) ALL Prescription medication must be in the prescription labeled container and name, dose, frequency must match the directions on the form. Over-the-counter medications must be in a small unopened container and directions should match the manufacturer’s instructions or a medical provider must sign the form. All medications and forms must be delivered to the school nurse No Later Than November 20, 2023, unless your child is suddenly prescribed a new medication. Dietary Restrictions and Needs
The permission form has a space for you to indicate dietary restrictions. Please indicate on the form. The Rock Eagle dining hall is very accommodating.

Groups/Cabin assignments
Students will be a part of the cabin assignment process by helping select one person they wish to room with on their designated team. Each team of teachers will create cabin groups of 20 students and 2 adults based on their requests. Class groups are designed by staff here at BHMS, and are different from cabin groups.
Communication while on the trip:
Cell phones do not work very well, if at all, at Rock Eagle. In the case of an emergency, you can contact the camp at 706-484-2834 and leave a message with the office during business hours. After business hours, you may reach the night guard at 706-484-2821. During school hours, you can also contact the school office, and they will reach out to us.

Turn in the medical authorization form and medicine to the school nurse as soon as possible.
Make sure you have everything necessary for the trip. (follow the packing guide)
