REPORTS
As you formed your groups, please help us by submitting an initial report in your folder that is on the Literacy drive using the same form as the 9 week report. Please make changes as your groups change.
LISD DYSLEXIA & 504 EVALUATION REPORT UPDATES
* Page 3 bottom of page – data is taken from parent, teacher and interventionists input (not standardized testing)
* Page 4 Yes and No responses are based on Standardized testing scores – Below 90% would indicate a YES response as a deficit taken from the three areas on CTOPP
* Bottom of page 4 Yes and No responses are based on Standardized scores taken from the WJIII for Decoding, Letter/Word, Reading Comprehension and Fluency
* Spelling is based on Standardized score on TWS-5
* Note changes to Page 5 – ALL data needs to be considered, both formal and informal data to make a determination regarding UNEXPECTEDNESS
* Referral to special ed recommendation has been eliminated
* Recommendation to RTI committee has been eliminated
* Comments section MUST provide justification to support the committees recommendation
* Students are IDENTIFIED as Dyslexic, but QUALIFY for services under 504
* Use updated LISD Dyslexia & 504 Evaluation Report found in MTA shared drive in the Handbook under referral forms
GENERAL TESTING GUIDELINES – REMINDERS
* Re-read Testing Manuals to review administration of each test’s particulars
* Review Basals and Ceilings for each test
* Use age equivalents for all scoring
* Use Standard Scores for reporting purposes
* Review set up for WJIII to be sure of consistency – Confidence band 68%
* See directions for WJ set up in MTA shared drive word doc
* Fill out top portion completely – use black ink
* Scores can be entered in pencil (be sure it is dark)
* Read testing instructions as given exactly, otherwise it will invalidate the scores
* Use suggested starting points, except in rare circumstances
* Do not round up ages
* All old record books and manuals should have been shredded
