Student Vaccination Requirements
In order to reduce transmission and ensure students can remain on campus in the safest possible environment and receive the best education possible, Los Angeles Unified is now requiring all students who are eligible for the COVID-19 vaccination to be fully vaccinated by Monday, January 10, 2022.
Proof of vaccination must be uploaded to the Daily Pass
How to upload external COVID-19 vaccination record
Students and families that have questions or concerns regarding the vaccine are encouraged to call our family hotline at 213-443-1300 and review our Frequently Asked Questions.
7821 Avalon Blvd, Los Angeles, CA 90003
If you were vaccinated at a mobile clinic, your record has automatically been updated. However, if you were vaccinated at a site outside of the District, we need your help with uploading your child’s record! After the external vaccination document is submitted via the Daily Pass, it will be reviewed and verified by the District’s Community Engagement team. Please allow 2-3 business days for the verification process.
Don’t forget, students aged 12 and older, who participate in extracurricular, must have their first dose no later than October 3, 2021, unless a medical exemption or conditional admission applies. In order to participate in in-person learning for the Spring term, all eligible students age 12+ must be fully vaccinated by December 19, 2021.
Mobile Vaccination Teams
Los Angeles Unified is committed to providing access to vaccinations for our students ages 12+ and staff. Mobile vaccination teams will begin visiting school sites beginning August 30, 2021. Appointments will be available beginning August 23, 2021. For more information click the following:

We strongly encourage all families to register for the parent portal.
