Parents/Guardians,
The Maryland Youth Risk Behavior Survey (YRSB) and Youth Tobacco Survey (YTS) will be
conducted at Chopticon High on November 18, 2021, during Period 2. Your student's class may be randomly chosen to complete this survey.
The survey was designed by the CDC to identify risk behaviors that may include safety
behaviors such as use of helmets and seat belts, depression, and mental health, use of tobacco, alcohol, or other drugs, nutrition and physical activity, and sexual behavior. The survey takes no more than one class period (45 minutes) to complete.
The survey is anonymous, and students will not put their names on the survey. Also, no school or student will ever be mentioned by name in a report of the results.
If you do not want your child to take part in the survey, please complete the opt out form your child will bring home for you to sign and return to his/her teacher, within 3 days prior to November 18.
Please contact me at vhsmith@smcps.org if you have any questions.
