From the School Nurse

Dear Parents,

Please return the green health cards as soon as possible to your child's teacher, Ms. Parks at the front desk or myself in the clinic (just down the hall from the front desk)

If your child has asthma, severe allergies or Seizures, please have your pediatrician or specialist complete a new action plan. These have to be updated each school year.

If your child has asthma or allergies requiring epinephrine or albuterol as a rescue, please bring the medications to the clinic so we have life saving medication in case of an emergency. There is a medication authorization form to fill out to accompany these medications. Or if you prefer your child to carry albuterol or epinephrine with them or in their backpacks, please complete the emergency medication form as well so we know where it is stored in case of emergency.

Lastly, if you have any clothing that you no longer need or will be donating, please consider a donation to the clinic. All sizes are needed, particularly shorts and pants.

As always, please feel free to contact me at ebarrington@csdecatur.net if you have any questions or concerns.
