5Essentials Parent Survey - Window is open from February 3-April 2
Dear Parent,
Beginning February 3, students, teachers, and parents across Illinois will have an opportunity to participate in the ninth annual statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of your child’s school. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school.
On behalf of the Illinois State Board of Education, the Illinois 5Essentials Survey will be administered online by UChicago Impact at the University of Chicago. The survey gathers data related to five indicators that can predict important student outcomes, including improved attendance and larger test score gains. These five indicators that affect and predict school success are:
Effective Leaders
Collaborative Teachers
Involved Families
Supportive Environments
Ambitious Instruction
Our district has chosen to provide the optional Parent Supplement survey. If at least 20 percent of parents complete this survey, a parent supplement report will also be generated. The parent data will not be reported on the State Report Card.
Your participation in the parent portion of the survey will help us understand the conditions at your child’s school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
The Illinois 5Essentials Survey for students and teachers will be conducted February 3, 2021 through April 2, 2021. The Parent Survey Supplement will be conducted during that same timeframe. Kindergarten Registration for the 2021-22 School Year
Welcome to Addison School District 4
Kindergarten Registration is for children who will be five years old on or before September 1, 2021 and live within the boundaries of Addison SD4
Beginning on March 8, 2021, parents may go into the www.asd4.org/registration/ website to enroll or register their incoming Kindergarten Student. All forms will be completed online!
In addition to the required online registration, parents must provide the following:
· 4 proofs of residency (one must be a mortgage, rental agreement or attestation)
· Original birth certificate
· Immunization/physical form (must be turned in before the first day of school)
Schools will be open to complete registration by turning in proofs of residency, birth certificate, etc., on the following two dates for incoming Kindergarten families:
· March 24, 2021 from 9am to 7pm
· April 10, 2021 from 9am to 2pm
Appointments for these two dates will be made after the online registration is complete or by calling the school.
If a language other than English is spoken in the home, the child must have an assessment of their English Language Proficiency to determine correct placement in either General Education or Dual Language. The district is sponsoring an assessment date at Indian Trail Junior High on April 17 from 9-2. When the online registration is complete and additional registration information is submitted, an appointment will be made to have the child screened for English Proficiency on that date.
Yearbooks - Limited Quantities Left
Did you order your 2020-2021 Indian Trail Junior High School Yearbook yet? It is not too late! A limited number of yearbooks will be available for purchase in the Indian Trail Main Office. The cost is $25 cash or check (made out to Indian Trail). Order yours today before they run out!
