REGISTRATION INSTRUCTIONS

⭐New to the school district?⭐ New student packets can be picked up at the admin. bldg.(309 W Main) M-Th 8-4pm.

Step 1. JMC Information Update

  • Login to JMC: https://knoxvillecsd.onlinejmc.com/ (username is the last name of the Parent/Guardian).

  • Click on the REGISTER FOR SCHOOL tab on the left sidebar for the 2017-18 school year. Click on Start/Continue registration process.
  • Complete the online registration questions. Choose the "NEXT" tab on the upper right.
  • Verify/update contact information (household, parent, and emergency contact). Verify information on each page and continue through the registration pages until you reach the "FINISH" button.
  • Be sure to jot down each StudentID # (student demographics screen). The student ID is necessary to have in order to pay for fees in Step 2. (The student ID is a 3 or 4 digit number.)


Step 2. Payment of Fees

  • Pay your registration fees*, add money into your student’s lunch account (auto-replenishment can also be setup) and/or purchase student and adult activity passes at www.payforit.net. *Registration fees are listed here.

  • New to PayForIt? Create and add your student(s) to your PayForIt account: https://www.payforit.net/UserCreate.aspx

  • If you believe your child is eligible for free or reduced* meals, please download and complete this form and submit to: KCSD Child Nutrition Director, 309 W. Main, Knoxville, IA 50138. If you need assistance completing the form, please email or call: Julie.Miller@kcsd.k12.ia.us 641-842-6551 ext 8. Forms can also be faxed to 641-842-2109. (*Reduced prices in PayForIt are seen upon checkout/payment screen.)

*Please note that an early bird discount of $5 has been applied in the PayForIt system. Early Bird discount ends August 2.


Step 3. FORMS** - Complete if required for your student.

HEALTH

  • ALL students PK-12: Annual Health History Form
  • Grades 6-12 (optional): Tylenol consent
  • 12th grade: NEW requirement is for TWO doses of Meningococcal vaccine
  • 9th grade: Proof of a dental exam. An exam within 1 year before to 4 months after enrollment in 9th grade is acceptable. Your dentist should have the form.
  • 7th grade: in addition to the Tdap requirement, the NEW requirement is for 1 dose of Meningococcal vaccine on or after 10 years of age and grade 7 if born after September 15, 2004.
  • 3rd grade: Proof of an vision exam. An exam within 1 year before and 6 months after enrollment in 3rd grade is acceptable. Your eye doctor should have the form.
  • Kindergarten:
  1. Health History Form
  2. Immunization Record with required Kindergarten vaccines
  3. Kindergarten Physical (and lead test)
  4. Dental exam (within 1 year of start of school)
  5. Vision exam (within 1 year of start of school)
  6. Optional for those with food allergies: Diet Modification Form
  7. Optional for those who need daily medication at school: Medication Administration Form
  • Pre-School:
  1. Health History Form
  2. Immunization Record with required Preschool vaccines
  3. PreSchool Physical
  4. Optional for those with food allergies: Diet Modification Form
  5. Optional for those who need daily medication at school: Medication Administration Form
